Upcoming classes in 2023 for Active Level I Expanded Functions:

For active expanded functions classes, the lecture portion is online and the testing is done on-site in class. Please note that you'll need to complete the lectures before coming to class. There will be a lecture completion form that you will need to bring with you to be able to take the testing.

How To Register:

To register, please follow the steps below in the order they are listed.

• Download and fill out the application....the Full Level 1 course includes all other skills listed on application that are also offered separately. It does not include the 4 CE only skills. ​

• Applications can be sent to [email protected] or mailed to Davidson Family Dentistry "attention Traci N" (8573 Urbandale Ave. Urbandale, IA 50322).  Payments can be mailed in or a credit card payment can be made over the phone to our office (515-279-3848).

• After your payment & application are submitted, the student's email invite will be sent with the links for the online portion of class and Traci Nimmo will follow up with you. 

***Class location is in the lower level of Davidson Family Dentistry. Please park in the lower level of Davidson Family Dentistry's parking lot.
